How It Works:

• Within 24 hours after purchasing your functional clinical assessment, you will receive an email with instructions on how to complete your client intake forms
• We will begin processing your assessment within 24 hours after receiving your completed paperwork.
• A link to your digital health assessment along with a link to schedule your functional evaluation will be emailed to you 1 week after receiving your completed paperwork.
